Reporting concerns about the quality of your education or training

You can use the online form below to notify us of serious concerns about a foundation trainee pharmacist currently enrolled in the foundation training scheme. The form can also be used to inform us of concerns about a pre-registration trainee technicians currently in training.
By notifying us of any fitness to practise concerns about a trainee, you will help with sharing information; particularly, about serious issues that could pose a danger to life or safeguarding concerns.
As the pharmacy regulator, we will only investigate serious concerns about pharmacists, pharmacy technicians or pharmacies in Great Britain, where patient safety could be at risk, or public confidence in pharmacy could be affected. This is to ensure safe practice in a pharmacy and to protect patients and the public.

If you are an employer, then you should also inform these the relevant Statutory Education Body (SEB) overseeing the recruitment, funding and quality management of the foundation year.

Most concerns can be dealt with quickly and effectively by the pharmacy’s own complaints process. So, we recommend you raise these types of concern internally within your organisation.
